Description:
Provides a social model program for the physically frail, mildly handicapped, socially isolated and mentally impaired.

Offers individualized assessment and plan of care, supervised group activities, nutritional snack and noon meal, limited showers and transportation, along with assistance with mobility, eating, toileting and hygiene. A limited number of subsidized participation fee slots are available for qualifying seniors.

Schofield Residence is a not-for-profit organization providing long term care services to elderly and disabled individuals as well as educational programs and respite for caregivers. Schofield operates a nursing facility that provides long term care and short term rehabilitation, an adult day health care program, a Long Term Home Health Care Program and a licensed home care agency. Specialized services for individuals living with Traumatic Brain Injury (TBI) and HIV AIDS are also available through Schofield. Schofield also offers health and wellness education programs through the Schofield Adult Wellness Center located at their Nursing Facility in the Town of Tonawanda.

Our team of trained caregivers assist with companionship, medication reminders, housekeeping tasks, meal planning & preparation, errands and incidental transportation. The Total Aging In Place Program is a managed long term care plan (insurance plan) and a provider of long term care services. The program is designed to help its members to continue to live safely in the community. The program provides and coordinates needed health care services that participants can receive at the program's Day Center or in their home/apartment such as: home health care, physical/occupational therapy, social work, nutrition, non-emergent medical transportation, home delivered meals, durable medical equipment, Optometry, Podiatry, Audiology, Dentistry, personal emergency response systems, and social day care. Total Aging can be paid for through Erie County Medicaid or Private Pay.

Its members include nursing facilities, assisted living, senior housing and community health service providers that include home health care, rehabilitation, dementia care, respite and other special services for the elderly. The association provides a "Community Guide to Senior Care" for caregivers wishing to learn more about long-term care services, and serves as a resource for individuals and community groups.
